Join us on Thursday, July 29, 2010 as local female riders take to the roads, stopping along the way at each of the eight Adirondack Community Physician Offices, where families and children are cared for each day. The ride is being organized in conjunction with our 10th Annual Children’s Miracle Network Radiothon on Lite 98.7 and updates on the ride will be provided to listeners throughout the day.

About our Hospitalist Program

About our Hospitalist Program

The Faxton St. Luke’s Healthcare hospitalists provide in-patient care from admission to discharge for patients who do not have a primary care physician and require hospital admission for non-surgical reasons. The hospitalists also accept referrals from primary care physicians who want to focus on their outpatient practices.

What are the responsibilities of a hospitalist?

  • Manage your treatment at the hospital
  • Coordinate and consult with specialists
  • Closely monitor your progress
  • Stay in close contact with you, your family and your doctor
  • Be available in case of emergency.

How will my primary care doctor know what happens to me while I am at the hospital?

Upon admission and throughout your hospital stay, the hospitalist communicates with your primary care doctor about your stay and lets your doctor know if you need follow-up care after discharge. The hospitalist also will prescribe the medicines you need to take when you leave the hospital. Your primary care doctor will take care of refills, as well as any care you may need after your hospital stay.

How do I contact the hospitalist if I want to meet with him/her?

Please tell the nurse that you have a question and the hospitalist responsible for your care will be notified. We find it useful for families to designate one person for us to contact for any updates or changes in a patient’s condition. Since a hospitalist sees patients in the emergency department, intensive care unit and throughout the hospital during the course of the day, he/she may see you at varying times of the day.

What should I expect if I am cared for by a hospitalist?

Your primary care doctor will share your medical records and information so the hospitalist team will know about your current health conditions, medical history and preferences. The hospitalist team will examine you and coordinate all of your care, including tests and treatments while you are in the hospital. At discharge, the hospitalist team will coordinate the transfer of your care to your primary care physician.

Andrew J. Faber, MD

Dr. Andrew J. Faber, is the medical director for the Hospitalist Program at Faxton St. Luke’s Healthcare. Dr. Faber holds a bachelor’s degree from Union College in Schenectady and his medical degree from New York Medical College in Valhalla, New York. He completed his residency in internal medicine at Gelsinger Medical Center in Danville, Pennsylvania. Dr. Faber has been a member of the Faxton-St. Luke’s Healthcare’s staff since 1986 and is board certified in internal medicine.

Anil Patel, MD

Dr. Patel received his medical degree from Pramukhswami Medical College in India. He completed his residency training at Overlook Hospital in Summit, New Jersey and is board certified in internal medicine. Goutham K. Malempati, MD Dr. Malempati received his undergraduate degree and medical degree from Mahadevappa Rampure Medical College in India. He is board certified in family medicine.

Lisette Alfaro-Berg, MD

Dr. Alfaro-Berg received her bachelor’s degree in biology from New York University and her medical degree from SUNY-Downstate Medical Center in Brooklyn. She completed her residency in internal medicine at the University of Texas Southwestern Medical Center in Dallas and is board certified in internal medicine.

Tatjana Mondom, MD

Dr. Mondom received her medical degree from University of Belgrade, School of Medicine in Yugoslavia and completed her residency in family medicine at St. Joseph’s Hospital in Syracuse. She is board certified in family medicine.

Sarmond Siddiqui, MD

Dr. Siddiqui received his medical degree from Nagpur University, Indira Gandhi Medical College and Mayo Hospital in India and completed his residency in family medicine at St. Elizabeth Medical Center in Utica. He is board certified in family medicine.

Lwin Moe Win, MD

Dr. Win received his medical degree from the Institute of Medicine in Yangon, Myanmar and completed his residency in internal medicine at Wyckoff Heights Medical Center in Brooklyn. During this time he also received his master’s degree in Health Education from Western Illinois University. He is board certified in internal medicine.

Carol Lareau, RN, FNP

Carol Lareau, RN, FNP, is a member of the Hospitalist Program at Faxton-St. Luke’s Healthcare. Carol received her RN degree from St. Luke’s-Roosevelt Hospital School of Nursing in New York and her family nurse practitioner certification from Community General Hospital in Syracuse. She is a member of the American Academy of Nurse Practitioners and New York State Association of Nurse Practitioners.

Chinmay Patel, MD

Dr. Patel received his medical degree from SMT NHL Municipal Medical College in India. He completed his residency training at the University of Buffalo and is board certified in internal medicine.
